Ident: fication of Occurrence:

Failure to have two (2) independent ECCS' subsystems operable Centrary to Technical Specification 3.5.2.

Conditions-Prior to Occurrence:

Mode 1 power operation (92%).

Description of Occurrence:

At 2245 during performance of SP-340 ECCS Pump Operability, it was. discovered that DHV-lll "B" decay heat pump discharge throttle valve, would not control flow in automatic. DHV-lll did respond in manual centrol; maintenance actions were initiated and operability was restored at 0330 on 17 September 1980.

Designation of Apparent Cause:

The' cause is -attributed to the roller nut not engaging the actuating cam for high alarm in the switch that operates the valve operator motcr.

Analysis of Occurrence:

- There was-no effect upon the general public health or safety.

Redundancy was maintained by the "A" decay heat loop.

Corrective Action

" The roller nut was adjusted and the cwitch was functionally checked. The history of this instrument has been reliable. No

_ general problem is identified, however, an engineering evaluation-of-DHV-110 and DHV-lli control method is in progress.

Failure-Data:

This is the third occurrence for DHV-111 and the twelfth event reported under this Specification.
