Occurrence Date: -17 September 1980

. Identification of' Occurrence:

Failure to have two independeut pairs of Auxiliary Building ventilation exhaust fans contrary to Technical Specification 3.7.8.1.

Conditions Prior.to: Occurrence:

- At.1400 during normal operation AHF-14C, Auxiliary Building ven-tilation exhaust fan failed. The system was shut down and then restarted using AHF-14B and AHF-14D.

Designation of Apparent Cause:

- The cause is attributed to failure of two blades in the fan assembly.

Analysis of' Occurrence:

There was no affect upon the general public health or safety.

Redundancy was maintained by fans-AHF-14B and'AHF-14D.

. Corrective Action:

The fan motor was removed, repaired, and reinstalled and a new fan assembly was installed. Both were satisfactorily tested on 24 September 1980 at 1200. Two engineering evaluatiens are in progress to determine generic concerns and possible solutions.

Quality Contrcl evaluations of physical characteristics of f an blade in use and in storage are being conducted. (Ref. LER 78-070/
