Section III.D.I.(a) 10 CFR Part 50. Appendix J requires that "After the preoperational leakage rate tests, a set of three Type t tests shall be performed, at approximately equal intervals during each 10-year service

period. The third test of each set shall be conducted when the plant is shutdown for the 10-year plant inservice inspections."
The Unit 210-year inservice inspection (ISI) period commenced on April 1, l
1977 and ended on July 3, 1987. During that period, containment integrated leak rate tests (ILRT) were completed at Unit 2 on November 15, 1975 December 18, 1982 and November 24, 1985. No ILRT was performed i
during the first Unit 210-year ISI outage, r

During the period of March 1989 through July 1989, Types B and C local leak rate tests were conducted on Unit 2 containment penetrations.
To provide reasonably satisfactory assurances regerding the current condition of containment penetrations, particularly during core alterations operations, please provide t
a.
the combined Types B and C results of these tests in units of SCCM, l
and b
the individual test rests, test dates and test acceptance criteria I
for all penetrations in the Unit 2 containment providing air-to-air direct access from insiae containment to outside the containment (including into penetration rooms).

Your application references STP-M-622 as providing the test methodology that necessitated the request for a surveillance interval extension.
