June 9, 1980 Identification of Occurrence:

Failure to have two source range neutron flux monitors operable while in Mode 6, contrary to Technical Specification 3.9.2.a.

Conditions Prior to Occurrenco:

Mode 6 Refueling Dew ription of Occurrence:

At 0400 on June 9,1980 while performing SP-112, calibration of the Reactor Protection System, it was determined that the count rate amplifier for "A" Channel Source Range Neutron Flux monitor was out-of-tolerance creating an event contrary to Technical Specification 3.9.2.a.

Investiga-tion revealed that the pulse amplifier card and output stage amplifier card were defective.

Maintenance restored operability and the source range monitor was placed back in service after successful completion of SP-220, Source Range Functional Tests During Refueling Operations.

Designation of Apparent Cause:

The cause of this event is attributed to a de-fective pulse amplifier card and output stage amplifier card.

Analysis of Occurrence: No effect upon public health and safety as no core alterations or positive reactivity changes were made during the time of this event.

Corrective Action

Defective cards were removed and replaced with a new calibrated module.

No further corrective action required.

Failure Data:

This is the second occurrence of this - type reported. However, a review of maintenance rec-ords revealed no previous. failure history for these components.
