G-Arkansas Power & Light Company Arkansas Nuclear One, Units 1 & 2 50-313/50-368 APPENDIX A NOTICE OF VIOLATION Based on the results of the NRC inspection conducted September 26-29 and October 7-8, 1977, it appears that certain of your activities were not conducted in full compliance with your Construction Permit or NRC requirements as indicated below:
1.
Criterion V of Appendix B to 10 CFR 50 requires in part that,
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings."
Contrary to the above:
a.
The receipt inspection of the Unit 2 fuel assemblies was O
performed in accordance with a procedure (2503.01) which v
did not contain all of the features required by Section 10.3.1 of 1.he NRC approved Quality Assurance Manual for Operations.
b.
The individuals performing the receipt inspection of the Unit 2 fuel assemblie.e had not been qualified and certified in accordance with the requirements of Section 10.4.1 of the NRC approved Quality Assurance Manual for Operations.
c.
Step 7.2.16 of Test Procedure 2.400.01, " Steam Generator Hydrostatic Test," was changed by the startup engineer during the performance of the test, but the change was not documented in accordance with paragraph 4.2.1 of Startup AdministrativeProcedure(SAP)2-12.
d.
Four uncorrected release exception items had not been placed on the punchlist as required by paragraph 4.3.3.2 of Startup Administrative Procedure (SAP) 2-7 " Resolution of System Problems."
This is an infraction.
