NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on February 7-10, 1978, it appears that certain of your activities were not conducted in full compliance with conditions of your NRC license DPR-51 as indicated below:
1.
Failure to Take Corrective Action for Discrepant Piping 10 CFR 50, Appendix B, Criterion V states in part, " Activities affecting quality... shall be accomplished in accordance with these instructions, procedures, or drawings."
The AP&L Quality Assurance Manual Operations, Rev. 4, Section 16,
" Corrective Action," paragraph 16.2.1 states, "When deviations, j
deficiencies, malfunctions, or other abnormal occurrences or con-ditions are encountered, they shall be reported to responsible Os authorities for review and disposition in accordance with Section 15 of the Program." Section 15 provides instructions for resolving identified deficiencies and for documenting their resolution.
C6 ntrary to the above, the licensee failed to follow the procedures stand above in that nonconfonnance reports containing disposition and corrective action for the discrepant piping rejected in radio-graphic test reports 2105, 2110, 2187 and 2113 were neither prepared nor appropriately dispositioned as specified in the Quality Assurance Plan.
This is an infraction.
