Occurrence Date:

Identification of Occurrence:

Failure to perform required surveillance prior to Core Alterations as required by Tech Spec 4.9.2(b).

6.

Conditions Prior to Occurrence:

Mode 6 refueling.

7.

Description of Occurrence:

l At 1100 it was discovered that the Source Range Nuclear Instrumentation i

portion of the Instrumentation Functional Tests during Refueling Opera-tion Procedure (SP-220) had not been performed within eight (S) hours prior to Core Alteration.

It was determined that SP-220 should have been performed prior to the time of plenum removal as defined by Tech Spec 1.12. An investigation of this event revealed that the Tech Spec definition of " Core Alteration" was mis-interpreted to mean fuel move-ment only.

Plenum removal was accomplished during 0000/0080 shift on 16 March 1978 and SP-220 was satisfactorily performed at 0900 on 16 March 1978 and met the criteria of eight (8) hours prior to fuel movement.

Designation of Apparent Cause:

The cause of this event was due to personnel error in that the defini-tion of Tech Spec 1.12 " Core Alteration" was interpreted to exclude plenum movement.

9.

Analysis of Occurrence:

There was no safety hazard to the plant or general public as the Source Range Nuclear Instrumentation Functional Test was satisfactorily performed.

(Source Range channels were completely functional prior-to and during the plenum removal.)

10.

Corrective Action

All concerned personnel have been given instructions as to the defini-tion of Core Alteration by speed letter.
