This letter refers to the routine safety inspection conducted by Messrs.
M. Banerjee, E, Collins, N. Dudley and D. Lew, for the period September 3,1989,
,

through October 7,1989, at the Oyster Creek Nuclear Generating Station. The
inspection consisted of document reviews, personnel interviews and observations
e
of activities. The enclosed report documents the inspection results.

Inspectors discussed the findings with you and members of your staff after the
L
inspection.
Paragraph 4.0 of the enclosed report discusses an event in which corrective
L
maintenance ori a nuclear safety related system was not performed in accordance
'
with written procedures or instructions appropriate to the circumstances.
In
this event, you performed troubleshooting and maintenance on the No. 1 Emer-
gency Diesel Generator utilizing your immediate maintenance short form process.
The immediate maintenance short form, however, did not specify th? replacement
of some components on the No. 1 Emergency Diesel Generator.
These components
'
were replaced without written procedures or instructions.
In addition, the
immediate rnaintenance short form process was not appropriate for the circum-
stances and complexity of the diesel generator troubleshooting and maintenance.

Your increased attention in this area is

required to ensure an appropriate level of controls is maintained on struc-
tures, systems ar.d components important to safety. Also, noted by this

inspection was informality ir. site activities.
This includes the acceptance

of informal technical direction in lieu of appropriate work control (paragraph

4.0), inappropriate manipulation and evaluation of a temporary modification

(paragraph 6.0), and an informal evaluation of a safety related breaker failure
(paragraph 30.0). These examples require your attention to ensure site policies
'
regarding formal controls are effectively implemented.
Based on the results of this inspection, it appears that some of your mainte-
nance activities on a diesel generator were not conducted in full compliance
