CONSTRUCTION INSPECTION
DOCKET NO. 050-0312
The attached report of a routine, unannounced inspection of construction
activities at the site of the subject project on June 27-30, 1972 is forwarded
for information. One item of nonconformance was identified during the
inspection. This item concerns inspection methods used by the Electrical
Department .mich classifies nonconformance problems as field inspection
correctfans to shortstop nonconformance reports. This is a direct violation
of the licensee's QA procedure. Inunediate corrective action is expected.
We confirmed that satisfactory corrective action has been taken on all of
the itema discussed in our letter to the licensee dated May 4, 1972 and that
the actions on apparent items of nonconformance were as described in the
,
licensee's response dated May 30, 1972.
The report contains the status of the following items:
1.
Investigation of Weld Root Pass " Imperfections".
(Section I)
2.
Cracks in Lateral Support Rings for Steam Generators (Section I and II,
Paragraph 8.)
3.
Control Rod Drive Pressure Tubes (Section I and II, Paragraph 10.)
A review of the NCR system showed a weakness in the implementation of
corrective action for nonconformances. While many of the NCRs (Nonconforming
Reports) written on discrepancies would not be considered "significan.:", the
lack of cause and corrective action determinations indicates a definite
weakness-in nonconforming material control. We take the position that
emphasis on such cause and corrective action mst be directed toward those
conditions, procedures and management systems that allowed the discrepancy
,
to occur, rather than reiterating a description of the fault and its
disposition which are covered elsewhere on the report form. SMUD agreed to
make a thorough review of their NCR system, including the categorization of
disposition actions as well as cause and corrective action. This item will
be reviewed further during the next inspection.

Sacramento Municipal Utility District (Rancho Seco) - An RO
~1nspection was conducted on June 27-30 to review electrical
installation work and welding in Class I systems and components.
One deficiency, involving the failure to properly review and
document departures from the electrical cable routing and
termination specifications, was identified and was incorporated
in a Regional enforcement letter.

Some of the pressure. tubes for tne control rod drives shipped
to Royal Industries, Santa Ana, California, for UT examination

were found to have thin walls.
B&W is preparing a topical
report for submittal to the AEC on this problem.

up on this matter.
