SUPPLEMENTARY INFORMATION

3. Report Date: 11-May 1978 *
4. Occurrence Date: 24 April 1978
5. Identification of Occurrence:

175 ft. Delta temperature indication out of calibration contrarf to Technical Specification 3.3.3.4.

6. Conditions Prior to Occurrence:

Mode 6 refueling.

7. Dsseription of Occurrence:

i At 1100 during the performance of the Meteorological Monitoring Instrumentation )

Calibration (SP-158) it was discovered that the 175 ft. probe of the Delta temperature '

instrument had failed. Both the 175 ft. and the 33 ft. probes were replaced and cal- <

ibrated using SP-158, and returned to service within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

8. Dasignation of Apparent Cause.

Cause of this event was due to the failure of the 175 ft. temperature probe.

9 21ysis of Occurrence:

No' hazard to the health and safety of the general public as-the 175 ft. and 33 ft.

temperature indications were available.

10. Corrective Action:

No corrective action necessary as new probes were installed in the 175 ft. sud 33 ft.

Delta temperature instrument.

11. Failure Data:

First occurrence of this event.
