5. Identification of Occurrence:

Radiation area monitor 4 inoperable contrary to Tech Spec 3.3.3.1, Table 3.3-6, Item 2(a). ,

6. Conditions Prior to Occurrence:

Mode 6 refueling. ,

7. Dsccription of Occurrence:

At 0600 on 8 April 1978 Radiation Area Monitor 4 (RMA-4), radiation monitor for the Spent Fuel Storage Pool Area gaseous activity monitor, was inoperable due to sample pump tripping. Investigation by control technicians revealed that the carbon vane sample pump was binding and tripping motor on overload. The carbon vane sample pump was replaced on 11 April 1978. Retest )f sample pump indicates that intermittent tripping still occurs, but is less freluent. Investigation into cause et tripping is continuing. As Tech Spec 3.3.3.1, Table 3.3-6, Item 2(a) identifies this Radiation. Monitor as the Fuel Storage Pool Area Gaseous Activity -

(\p, Ventilation System Isolation Monitor, when in fact RMA-4 does not perform th 4

function. A requesg' for Tenh Spec change will be initiated.

During the information search of this report it was also discovered the RMA-4 was inoperable on 31 March 1978. Further investigation revealed that Operations personnel had reviewed Tech Spec 3.3.3.1, Table 3.3-6, Item 2(a) on 31 March 1978 and determined that RMA-4 would meet the criteria set forth in Tech Specs and RMA-4 inoperability was not a nonconforming operation.

8. Dctignation of Apparent Cause:

Apparent cause of RMA-4 sample pump tripping is unknown at this ti=e.

9. Analysis of occurrence:

There is no hazard to the health and safety of the plant or public as final effluent

=onitor RMA-2 is operational.

10. Corrective Action:

RMA-13 was put in operation to monitor spent fuel pool area gaseous activity. Pro-posed Tech Spec change is forthcoming to omic RMA-4 as a required monitor. Repair of RMA-4 will be accomplished when investigation is complete. Operations personnel have been cautioned by speed letter that any time there is a question as to Tech Spec applicability, a Nonconforming Operation Report will be submitted.
