0510 5/25/80 Identification and' Description of Occurrence:

FCV-63-73 would not open during the performance of SI-166.3.

RHR pump 1B-B and containment spray pump 1B-B are inoperable with this valve inoperable.

Conditions Prior to Occurrence:

Unit 1 in Mode 4 Apparent Cause of Occurrence:

Failure of FCV-74-21 to make up its limit switch caused the interlock between FCV-63-73 and 74-21 to not be made up.

Analysis of Occurrence:

There was no danger to public health and safety.

Corrective Action

Emergency maintenance request was written to correct the limit switch problems with FCV-74-21.

Failure Data:

Previous occurrences - none.
