Date of occurrence: 5/30/80 Time of Occurrence: 0800 EST Identification and Description of Occurrence:'

The listed Westinghouse supplied level transmitters (Foxboro Model E13DM-HSAH1) located inside containment in accumulator No. 4 room do not have the nece.ssary qualifications to withstand a post accident environment.

TVA Tag Fos. LT-3-43, -56, -98, -111 Qi Nos. LT-501, -502, -503 -504)

Condition Prior to Occurrence:

Unit in Mode 5 with all fuel loaded.

Action specified in the Technical Specification Surveillance Requirements met due to inoperable equipment.

N/A Apparent Cause of Occurrence:

Per FSAR Table 3.11-2 the temperature could reach 327 F, the instruments are 0

qualified up to 180 F operation.

Analysis of Occurrence:

Should the post accident environment of the No. 4 accumulator room reach 0

or exceed 180 F, the instruments possibly would not be capable of performing their intended post accident functions.

There was no hazard to public henith and safety.

. Corrective Action:

The subject transmitters are to be replaced with transmitters capable of withstanding the accident environment.
