NOTICE OF VIOLATION isased on the results of an NRC inspection conducted September 12-15, 1977, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
1.
Arkansas Nuclear One, Unit One, Technical Specification 6.5.1.6 requires the Plant Safety Committee (PSC) to invesi.igate all violations of the Technical Specifications and to forward a report of their evaluation and recommendations to the Superintendent of the Power Plant.
Contrary to the above requirement, the PSC failed to investigate and submit the required report for Infraction 1 identified in Inspection Report 50-313/77-10 and 50-367/77-11.
This is an infraction.
2.
Criterion V of Appendix B to 10 CFR 50 requires that " activities l
affecting quality shall be prescribed by documented instructions, procedures... and shall be accomplished in accordance with these instructions, procedures...."
Contrary to the above requirements, nine examples were found of the-applicant's failure to document the installation of temporary jumpers and bypasses in accordance with his Startup Administrative Procedure SAP 2-9 at Arkansas Nuclear One, Unit two.
This is an infraction.
