v License No. DPR-72 APPENDIX A Notice of Violation Based on the results of the NRC Inspection on September 20-23, 1977, it appears that certain activities were not conducted in full compliance with NRC regulations and the conditions of your NRC Facility License No. DPR-72 as indicated below. These items are deficiencies.
A.
Section 5.6.2.A of the Environmental Technical Specifications (ETS) states that any unplanned release of radioactive material from the site shall be reported to the NRC Office of Inspection of Enforcement within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.
Contrary to the above, a gas release that occurred at 0716 on September 3, 1977 while venting the makeup tank was not reported to the NRC until 1200 on September 4, 1977.
B.
Technical Specification 6.8.1 requires that written procedures as recoimeended in Appendix "A" of Regulatory Guide 1.33, November, 1972 shall be established, implemented and maintained.

Contrary to the above, procedures of AP-103 (Radiation Monitoring System Alarms) which require immediate termination of a reactor building purge in the event of a failure of the purge monitor were not followed. A purge monitor failure alarm was received at 0509 on September 18, 1977 but the reactor building purge war not terminated until 0800 on September 18.
