Description of Occurrence:

While reviewing the complaced Containment Entry Surveillance Procedure SP-430, it was discovered that the Data Sheet #1 (Airlock Pressure Test) had not been completed for a Containment entry on 12 December 1977.

Further review indicated that the Containment was entered on 15 December 1977 and the Airlock Pressure Test was satisfactorily performed.

There were no safety implications as the results of the complaced Airlock Pressure Test on 15 December 1977 were satisfactory.

10.

Corrective Action

A revision to the Master Surveillance Plan, SP-433, requiring an Airlock Pressure Test be performed daily regardless of Containment entry, has been entered.

11.
