4. Occurrence Date: 12 December 1977 (discovered 29 December 1977)
5. Identification of occurrecca:

The failure to demonstrate the operability of both Conts'=nt airlocks contrary to Technical Specification Surveillance 4.6.1.3.a.

6. Conditions Prior to occurrence:

Mode 1 operation.

7. Description of Occurrence:

While reviewing the complaced Containment Entry Surveillance Procedure SP-430, ',

it was discovered that the Data Sheet #1 (Airlock Pressure Test) had not been .

completed for a Containment entry on 12 December 1977. Further review indicated that the Containment was entered on 15 December 1977 and the Airlock Pressure Test was satisfactorily performed.

Designation of Apparent Carse:

The cause of this occurrence was the failure to follow prescribed procedures, in that SP-433 requires Airlock Pressure Test be performed daily in Modes 1-4 if the Containment has been entered.

9. Analysis of occurrence:

There were no safety implications as the results of the complaced Airlock Pressure Test on 15 December 1977 were satisfactory.

10. Corrective Action:

A revision to the Master Surveillance Plan, SP-433, requiring an Airlock Pressure Test be performed daily regardless of Containment entry, has been entered.

11. Failure Data:

This is the second occurrence of missed Airlock Pressure Test as reported by LER 77-119.
