Luke Power Cc pany September 10, 1971 Within these areas, the inspectics censisted of selective examinations of precedures and representative records, interviews with personnel, and ob-servatiens by the inspectors.
Based en our review of the actions ycu have taken with respect to items identified in your letters Of April 22 and June 13, 1971, we have no further questions on the identified items ; hcVever, these ite s which were in the process of correction vill be given additional attentien during our next inspection.
During the inspection, it ves found that certain of your activities appear to be in nonce pliance with Appendix 3 to 10 CFR 50 of the AIC egulations,
" Quality Assurance Criteria for :Tuclea ?cver Plants." 2.e items and refer-ences to the pertinent requirements are listed in the enciesure to this letter. Please provide us within 30 days, in writing, with your cc ents ccncerning these iters, any steps which have been er vill be taken to ccrrect then, any steps which have been er vill be taken to prevent re-currence, and the date all ecrrective actions er preventive measures were or vill be ec=pleted. Your reply should emphasice, in particular, any appropriate changes that have teen or vill be made to i= prove the effec-tiveness of your quality assurance progren.
Frc: our discussions, it is our underster. ding that Duke vill (1) evaluate a pcssible flav in a Uni 2 pu=p casting, (2) 'urther evaluate radiographs of portiens of the Unit 2 core injection and core ficoding piping which

Instructions, Procedures, and Drnvines Criterion V states, " Activities affecting quality shall be pre-scribed by doeurented instructions, procedures, or drawings, er a type appropriate to the circu: stances and shall be acec plished in accordance with these instructiens, procedures, er drawings..
2.e second paragraph of 3s*4 Precedure FS-1-2, " Procedures By Installing Centracter," states, "Frior to handling any B&*4 supplied equipment, the Erecting Centracter is requested to reviev the content of this manua' and to ma%e available to the B&'4 Site Representative for review, any written plans, drawings, etc., in-dicating hev reqairements of this manual vill be met."
Centrary to the above, the Duke censtruction department proceeded vith the installatien of the feedvater headers en steam generator 23 in Unit Zio. 2 without a vritten approved procedure, and without direct supervision ? d qualit;* centrol surveillance during critical operations.
2.e gas cutting o,erations as conducted on the header support lugs and the steam generatcr supperts did not adhere to a 3&*4 vaming en the stecs generator against cutting and are strikes on this coded pressure vessel.
