Dear Mr. O'Reilly,

Attached are our responses to the reference report which summarizes Mr. Jack Skolds' inspection February 1 through February 29, 1980. Specifically, we have addressed the infraction dealing with Startup Procedure SUM-B-12, " Lifted Lead and Jumper Control" where two leads with " Lifted Lead" tags attached to them were not in fact " lifted". We consider that the actions that we have taken are appropriate to assure that this type of event will not recur.

Description of Event 1 l

On February 2,1980, while touring the Control Room the NRC resident i inspector noted that leads 9 and 10 on Terminal Board 730 on the Radiation

Monitoring Panel had lifted lead tags attached to them. However, the leads 1 were not lifted. Lifting of the leads is normally accomplished by using )

" standoffs" made of an insulating material. These standoffs were not present and the leads were attached to the Terminal Board. Startup Manual Procedure SUM-B-12 " Lifted Lead and Jumper Control", paragraph 6.2, states in part that temporary alterations are made as identified by the " Nature I

of Alteration" on the Lifted Lead / Jumper Tag. The temporary alteration (lif ted leads) called for on tag number 679 did not exist in that the leads were not lifted.

Probable Consequences There are no adverse consequences since the purpose of the temporary )

alteration was for testing purposes concerning Radiation Monitoring System circuitry to prevent actuating valves and ventilation equipment associated )

with the circuitry. No tests associated with the subject temporary alter- l ation were performed during the affected period, therefore no equipment was 1 inadvertantly actuated.

i Cause of Event On January 8, 1980, a 100% audit of the Lifted L;sd and Jumper Log was I completed. The results of this audit showed that the subject temporary alteration was in satisfactory condition. Due to the large number of 1 personnel working in the Radiation Monitoring Panel, the subject leads were inadvertantly re-instated to an operable status by an unidentified

? individual.

l The subject temporary alteration on leads 9 and 10 on Terminal Board 730

a. Require independent verification by Operations personnel of proper alteration and tag placement,
b. Require the Shift Supervisor to perform a que.rterly audit of the "Lif ted Lead / Jumper Record".

In addition, all Shif t Supervisors have received instructions on the revised Virgil C. Summer Nuclear Station Startup Manual Procedure SUM-B-12, " Lifted Lead and Jumper Control", to insure knowledge of their responsibilities and duties concerning the implementation of the Lif ted Lead / Jumper program.

Applicable Startup, Construction; and Maintenance personnel have been instructed to be cognizant of their responsibilities and duties concerning the Lif ted Lead and Jumper Procedure.
