'8005070 APPENDIX A NOTICE OF VIOLATION South Carolina Electric a'nd Gas Company License No. CPPR-94 V. C. Summer Nuclear Station Based on the NRC inspection February 1-29, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by 10 CFR Appendix 9, Criterion V implemented by South Carolina Electric and Gas Company FSAR Section 17.1.5, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings...and shall be accomplished in accordance with these instructions, procedures and drawings". Paragraph 3.5 of Administrative Procedure 204.1, Safety Tagging states in part that safety tags shall be attached to the equipment valve, switch or device named on the tag.
Start Up Manual Procedure SUM-B-12
" Lifted Lead and Jumper Control", Paragraph 6.2 states in part that temporary alterations are made as identified by the " Nature of Alteration on the Lifted Lead / Jumper Tag.
Contrary to the above, activities affecting quality were not accomplished in accordance with established procedures in that:

The following safety tags were not attached to the breaker named on the tag on February 7, 1980:
Tag Number 1613 for 16EH (IDBZY (spare) was placed on the breaker for valve 8808B Tag Number 1768, 1561, 1068, 1122, 10-281-79-1, 12-046-79-5, and 08-276-79-1 for the breaker for valve 8811A were located on a spare breaker on Panel IDA2Y Tag Number 1536 for a spare breaker on Panel IDA2Y was located on the breaker for valve 8811A.

The following safety tags were not attached to the valves named on the tag on February 27, 1980:
Tag number 2421 for valve 8524A was located on valve 8542A.
Tag number 2422 for valve 8524B was located on valve 8542B.
3.
The temporary alteration on leads 9 and 10 on Terminal Board 730 on the Radiation Monitoring Panel w'ere not made as.(dentified by the
" Nature of Alteration" on Lifted Lead Tag Number'679 in-that the leads were not lifted on February 2, 1980. -
This is an infraction. A similar item was brought to your attention in our letter dated January 18, 1980.
