Containment isolation v:<lve FWV-46 Inoperable contrary to Technical Specification 3.6.3.1.

Conditions Prior to Occurrence:

Mode 5 cold shutdown.

Description of Occurrence:

At 0100 during the repair of "B" OTSGifeedwater swing-check valve, FWV-46, it was discovered that the valve disc retainer pin was missing. The loss of the pin prevented the check valve disc from closing. Repair actions were initiated.

Designation of Apparent Cause:

The cause of this event is attributed to a deficiency occurring during the initial assembly of the valve by the manufacturer.

It was determined that the set screw, which locks the hinge pin in place, was not fully inserted.

Analysis of Occurrence:

There was no hazard created for the plant or the general public as a result of this event. The plant was maintained in a stable cold shutdown condition.

Corrective Action

Modification 80-4-63 was initiated to upgrade the retainer pin assembly. The

'dification was installed on 8 April 1980 with post modificati. testing scheduled to be completed upon restora-tion of the feedwater system.

In addition, OTSG feedwater swing-disc check valve FWV-45 will be inspected to identify similar concerns in this area.

Failure Data:

This is the first occurrence of this type reported.
