This letter relates to the discussions Fr. Kelley of this office held with Messrs. Bennett and Frcats during and at the conclusion of the inspection O

conducted on February 9 through 12, 1971, regarding the construction activ-ities authorized by AEC Construction Per=1t No. CPPR-51 for the Crystal k

River facility.

As noted during the discussions, apparent deficiencies were identified in-volving an item not in cenformance with Appendix B to 10 CFR 50 cf the AEC Regulations, " Quality Assurance Criteria for Nuclear Power Plants." The item it as follows:

The Code of Federal Regulations, 10 CFR Part 50, Appendix B, QA Criterion V, requires that activities affecting quality shall be prescribed by docu=ented instructions, procedures or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures,or drawings.

Instructions, procedures,.or drawings shall include appropriate quantitative or qualitative acceptance criteria for deter =ining that i=portant activities have been accomplished.

Contrary to the above, the FPC QA progra= failed to function

adequately as indicsted by the following: The toriconical section of the containment liner was erected by CELI with the top edge above and below the elevation 103'-0" stated on the erection drawings. The erection of the cylindrical courses continued after trimming that portion of the toriconical section above the 103'-0" elevation leaving two gaps 220 of the circu=ference with a height of up to two inches.
