5. Identification of Occurrence:

Contrary to Environmental Technical Specification 3.1.5, a sample was not obtained from test well 5 for the months of April and May.

6. Conditions Prior to Occurrence:

N/A

7. Description of Occurrence:

The April and May samples from test well 5 were not collected because the well i had been inadvertently covered during construction in the area. g

3. Designation of Apparent Cause:

'4h11e repairing a pipe break in the area, debris was placed on top of test well

5. The well is only a pipe that extends about one foot above the ground. During cleanup after the repair, the debris and the well were leveled.
9. Analysis of occurrence:

The closing of the well did not result from operation of Crystal River Unit 13 nor is operation of the plant affected by it.

10. Cor ective Action:

Upon discovery, neasures were instituted to ensure that the well would be available for sampling in June (as the sampling period for May had already passed). The well was re-established for sampling on 26 May 1973. In addition, narkers will be attached to the well for higher visibility to preclude this type of accident in the future.

This is the firs. report for this type occurrence.
