Identification of Occurrence:

Containment isolation valve ARV-1B failed to close upon RM-Al high radiation alarm contrary to Technical Specification 3.6.3.1, Table 3.6-1.

Conditions Prior to Occurrence:

Mode 1 power operation.

Description of Occurrence:

At 1745, a Reactor Building purge was initiated.

Reactor Building purge monitor 5

RM-Al alar:ned high and the containment purge exhaust valve AHV-13 did not close.

The valve was closed by using the control switch on the ES control panel.

Redundant valve AHV-1A operated as designed. Investigation revealed that valve AHV-13 had been opened slightly when initiating the purge and had not activated its open limitsvitch.

The closure matrix was not complete and when RM-Al high alarmed the closure of AHV-13 was not effected. With purge supply valves AHV-lc and AHV-lD closed and purge exhaust valve AHV-1A closed, valve AHV-13 was fully opened. The valve was closed by iniciating a high alarm on EM-A1.

The closure was timed and the results of closing and timing were satisfactory and the valve was restored operable.

Designation of Apparent Cause:

The cause of this event was due to purge exhaust valve AHV-13 not being opeo enough i

to activate its automatic closure matrix.

Analysis of Occurrence:

There was no safety hazard to the plant or general public as redundant purge exhaust valve AHV-1A was operable and closed as designed.

Corrective Action

A request for Engineering Information (REI) has been submitted to investigate and evaluate this occurrence and to recommend action to ensure auto closure of valve upon receiving a high radiatica alarm from RM-A6.

This is the first occurrence of this event.
