2. Facility: Crystal River Unit #3
3. Report Date: 28 August 1978 4 Occurrence Date: 2 August 1978
5. Identification of Occurrence:

Chemical Addition Pumps, CAP-1A and 13 inoperable contrary to Technical Specification 3.1.2.1 and 3.1.2.6.

6. Conditions Prior to Occurrence:

Mode 6 refueling.

7. Description of Occurrenca:

At 0030, on 2 August 1978 it was discovered that the Chemical Addition Pumps, CAP-1A and 13 failcl to develop s dficient discharge pressure while performing Surveillance Procedure SP-320, 0; rability of Boroc Injection Sources and Pumps.

The action statement requiring thz suspension of all operation involving core alterations or positive reactivity changes until at least one injection path is restored was immediately instituted. The Borated water storage tank was available but contained a concentration not yet within Technical Specification requirements to perform core alterations. The pump case and suction lines were vented. Within thirty ninutes system opert 111ty was restored and SP-320 was successfully accomplished meeting acceptance criteria.

8. Designation of Apparent Cause:

An investigation is under way to determine the cause of this anomaly.

9. Analysis of Occurrence:

This event had no safety significance as there were no operations involving '

core alterations or positive reactivity changes at the time of this event.  ;

10. Corrective Action: I Short Term Instructions have been issued to run the Chemical Addition Pumps, CAP-1A and 13 each day to reduce the problem of the pumps becoming air bound and losing auction pressure until the investigation is complete and data is analyzed, at which time a more positive corrective action can be established.
11. Failure Data:

This is the third occurrence of an event of this type.
