9 LER CONTINUATION -- RO # 2-79-33 Facility: BSEP l' .it #2 Event Date 5-18_7,9 technical manual and the turbine mechanical overspeed trip was tested several times.

The Terry Turbine representative stated that if the set screw is correctly installed, it will lock with the threads of the spring adjusting screw and will not be able to back out. The turbine has been run several times since with no problems. The setscrew and the spring adjusting screw on both units were checked during the past refueling outages with no discrepancies found. This is considered an isolated incident and no further action is required.

Through an administrative deficiency, this event was not detected until May 18, 1979. The HPCI system was under an LCO for a non-reportable reason. When additional problems developed that were reportable, they were maintained under the same LC0; therefore, the reportable problen was not reviewed by management. An administrative procedure is being written to ensure that all such occurrences are identified, reported, and properly reviewed.
