a SUPPLEMENTARY INFORMATION Report No.* 50-302/79-052/03L-0 Facility: Crystal River Unit #3 Report Date: 7 June 1979 Occurrence Date: 16 May 1979 Identification of Occurrence:

The Fire Suppression System inoperable contrary to Technical Specification 3.7.ll.l.a.

Conditions Prior to Occurrence:

Mode 6 refueling.

Description of Occurrence:

At 1300, it was discovered tha both diesel driven fire service pumps, FSP-2A and 2B, were inoperable. Investigation revealed that a clearance had been issued for the yearly inspection of Fire Service Pump FSP-2B. However, the diesel manufacturer rep-resentative, in an effort to avoid delay, disconnected the starter on FSP-2A without meintenance personnel assistance and proceeded to perform SP-606. Maintenance personnel assigned to the job arrived at the jobsite and assisted the Technical Rep-resentative. During the performance of the SP-606 it was dis-covered that FSP-2A was not under clearance. Supervisory personnel were notified and FSP-2B was returned to service at 1315.

Designation of Apparent Cause:

The cause of this event is attrib 2ted to the manufacturer's representative not having adequat.e training to follow the Clearance Procedure.

Analysis of Occurrence:

No effect upon public health and safety. Fire Service Pump FSP-1 was available and operable.

Corrective Action:

A Discrepancy Report #5873 was initiated as a result of this event.

Personnel involved were instructed on the importance of adhering to approved procedures and on proper maintenance practices. No further corrective action required.

Failure Data: 2354 008 This is the first occurrence of this type reported.
