O'Reilly, Director April 12, 1979 IE INSPECTION REPORT 50-369/79-14 APPENDIX A NOTICE OF VIOLATION Duke Power Company License No. CPPR-83 McGuire Unit 1 Based on the NRC inspection February 21-23, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below.
These itema have been categorized as described in correspondence to you dated December 31, 1974.
As required by Criterion V of Appendix B to 10CFR 50 and implemented by Topical Report on Quality Assurance, Duke 1-A, work performed on safety-related items shall be accomplished in accordance with prescribed instructions, procedures and drawings. Duke Power Company (DPC) drawing number MC 1785-09.01 specifies the termination points for the two conductors of cable 1*NC752 at flow transmitter INCFT 5050.
Contrary to the above, cable 1*NC752 was not terminated in accordance with DPC drawing MC 1785-09.01 at flow transmitter INCFT 5050.
The transmitter circuit had been released to DPC's Steam Production group.
This is a deficiency.
Response
Frcm our investigation of this deficiency, it appears that the cable lead reversal on flow transmitter INCPT 5050 was an isolated incident. Normally, this type of incorrect termination would be detected by the Construction Department electrical checkout or by the Steam Production Department calibration and fune-tional test.
Iloweve r, in this case the instrument leads were also reversed on the terminal block so the transmitter functioned properly.
A Random Injection Worksheet (QA Form M-40c) serial E-895 has been laitiated which requires that the termination leads reversal be corrected. Additionally, all saf ety-related terminal blocks inside Containment are being replaced with environ-mentally sealed splices, which should minimize the possibility of this type occur-rence in the future.
