19 January 1979 Identification of Occurrence:

One inoperable engineered safety feature actuation system instrument channel contrary to Technical Specification 3.3.2.1.

Conditions Prior to Occurrence:

Mode 1 power operation (68%)

Description of Occurrence:

At 1346 while performing the Engineered Safeguardc Monthly Functional Test (SP-130), it was discovered that RC3-GP3-BL4 light would not extinguish when the test switch was returned to the off position. Upon investigation it was determined that relay AR had failed. Operability was restored in two hours.

Designation of Apparent Cause:

The cause of this event was found to be dirty contacts in the AR relay.

Analysis of Occurrence:

There was no hazard to the plant or general public as the affected channel was in a test mode with the two remaining channels operable.

Corrective Action

The relay was sprayed with a contact cleaner, tested, and returned to operable status.

Engineering study (PD-718) associated with air filtration of engineered safeguards cabinets is being conducted to prevent recurrence.

Failure Data:

Eleven previous occurrences reported.

Fourth occurrence of this type.
