23 May 1979 Identification of Occurrence:

Both boron injection flowpaths inoperable contrary to Technical Specification 3.1.2.1.

Conditions Prior to Occurrence:

Mode 6 refueling.

Description of Occurrence:

At 0330, while performing Surveillance Procedu'e SP-320, Opera-bility of Boron Injection Sources and Pumps, it was discovered that Heat Trace Recorder, HTRT-5, which monitors the concentrated boric acid flowpath, was not printing. This rendered the con-centrated boric acid flowpath inoperable.

The borated water storage tank supply was also inoperable due to the boron con-centration being 21 ppm below the Technical Specification limit of 2270 ppm.

The heat trace recorder HTRT-5 was restored operable through maintenance at 1745 on 25 May 1979.

Designation of Apparent Cause:

The cause of this event is attributed to a loose recorder drive cord.

Analysis of Occurrence:

No effect upon public health and safety as the plant was in a safe shutdown condition. Core alterations were not in progress during the time of this event.

Corrective Action

The drive cord was replaced and HTRT-5 was returned to service following satisfactory completion of SP-320.

Request for Engineer-ing Information 78-10-4 to incorporate independent concentrated boric ac.t suction and discharge lines, and 79-5-1 to evaluate recorder reliability, have been submitted.

Further corrective actions will be based upon the results of these evaluations.

Failure Data:

This is the third occurrence reported for HTRT-5.
