Trip bistables for pressurizer pressure transmitter PT455 had not been tripped before closing the pressurizer level transmitter LT459 root valves (common with PT455).

Conditions Prior to Event:

The unit was in Mode 1.

Description of Event:

At 0330 on 11/7/79, it was determined that trip bistables for pressurizer pressure transmitter PT455 had not been tripped before the LT459 root valves (common with PT455) were closed.

This is being reported under Tech. Spec. 6.9.1.9.c. The root valves common to both PT455 and LT459 were closed at 2145 on 11/6/79 for calibration checks (trip bistables for LT459 were already tripped). The root valves were reopened at 0145 on 11/7/79 following completion of LT459 calibration checks. Health and safety of the public were not affected.

Designation of Apparent Cause:

This occurrence was caused by personnel error. Certain surveillance test procedure requirements i.e. tripping of PT455 bistables when LT459 root __

valves are to be closed, were not met due to personnel involved failing to perform one step in the initial conditions.

Analysis of Event:

At 2145 on 11/6/79 the root valves common to both PT455 and LT459 had been closed for LT459 calibration checks. Trip bistables for LT459 were already tripped as required by an LCO already in effect. However, trip bistables for PT455 were not placed in the tripped condition before closing the root valves. Administrative controls were in effect to ensure sign offs of I & C STP prerequisites e.g. bistables being tripped before root valves are closed. However, Operationa and I a C personnel involved in the incident overlooked one step of the initial conditions

• and as a result the bistables for PT 455 were not tripped.

LER 79-050/01L-0 Corrective Action:

This incident will be reviewed with I & C and licensed Operations personnel to point out the effects of the errors involved in this occurrence and personnel will be reinstructed on the administrative controls that should have prevented this incident from occurring.
