16 May 1979 Identification of Occurrence:

The Fire Suppression System inoperable contrary to Technical Specification 3.7.11.1.b.

Conditions Prior to Occurrence:

Mode 6 refueling.

Description of Occurrence:

At 0100 while performing Daily Shutdown Surveillance SP-301, it was discovered that the Fire Service Tank (FST-1B) volume was 3,600 gallons below the minimum Technical Specification require-ment of 345,000.

FST-1B was refilled to within specification and was declared operable at 0700 on 16 May 1979.

Designation of Apparent Cause:

The cause of this event is attributed to improper design.

The low level alarm setpoint was established below the level required to meet Technical Specification requirements.

Analysis of Occurrence:

No effect upon public health and safety. Alternate water supplies were available from Fire Service Tank FST-1A and Units 1 and 2.

Corrective Action

Modification 79-3-63, initiated as a result of previous occurrences, raises the low level alarm setpoint above the Technical Specifica-tion limit.

This modificatior was completed 5 June 1979. No further corrective action required.

Failure Data:

This is the fourth occurrence reported.

Reference LER's 79-15 and 79-42.
