N' APPENDIX A NOTICE OF VIOLATION Florida Power Corporation License No. LPR-72 Crystal River ~1 Based on the NRC inspection October 22-26, 1979, certain of your activities were apparently not conducted in full compliance with h7C requirements as irficated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
Technical Specification 6.8.1.a specifies in part that written procedures shall be established, implemented, and maintained covering the activities referenced Selow in Appendix A of Regulatory Guide 1.33 Noveuber 1972.
Pr;cedure adherence requirements of item 1.d of Appendix A were established in Section 2.7 of Staticn Administrative Instruction AI-500 which requires that when an operator has performed an operation he initials the space provided for that step in the procedure or on a Check Off List.
Contrary to the above, on October 19, 1979 part II-A of radioactive release permit RBVRP-79-55 was signed indica *.ing completion of step 3.3.1 of Radio-active Airborne Release Permit Procedure, RP-105.
This step was to adjust and verify setpoint and alert 1cvels for radiation monitors in accordance with values specified on the permit. However the actual settings were ten times those on the permit.
