10 May 1979 Identification of Occurrence:

One (1) neutron flux monitor (NI-2) inoperable contrary to Technical Specification 3.9.2.

Conditions Prior to Occurrence:

Mode 6, refueling.

Description of Occurrence:

In Mode 6, at 0800, while calibrating the Reactor Protection System through Surveillance Procedure SP-ll2, it was dis-covered that the rate of change amplifier and the count-rate amplifier for Nuclear Source Range Instrument NI-2 were out of tolerance. Calibration restored operability and NI-2 was returned to service at 1345 on 5/10/79.

Designation of Apparent Cause:

The cause of this event is attributed to instrument drift in the count-rate and rate of change amplifiers.

Analysis of Occurrence:

No effect upon public health and safety as the plant remained in a safe shutdown condition.

Core alterations were not in progress at the time of this event.

Corrective Action

The count-rate amplifier and the rate of change amplifier were calibrated and NI-2 was returned to service at 1345 on 5/10/79 following satisfactory completion of SP-ll2.

No further corrective action is required.

Failure Data:

This is the second occurrence reported for NI-2.
