APPENDIX A NOTICE OF VIOLATION Florida Power Corporation License No. DPR-72 Based on the NRC inspection September 10-15, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as in-dicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by Technical Specification 6.8.1, in part, " written procedures shall be established, implemented and maintained...".
Crystal River Plant Operating Quality Assurance Manual Control document AI-400, states, in part, "5.2, All data or calculation sheets which are portions of a proce-dure must be completed while the task is being performed.
Each data sheet shall be initialed and dated by the person or people taking the data and approved by the responsible supervisor...".
Contrary to the above, the following examples of nonconformance to quality record-keeping and review were identified during this aspection:
1.
SP-135, "ES Actuation System Response Time", reviewed May 3, 1979 had several data table pages filled out, initialed, but not dated as to when the tables were completed.
2.
SP-102, " Control Rod Drop Time Test", reviewed July 31, 1979 did not have Data Sheet II initialed or dated as 'o its performance.
3.
SP-103, " Moderator Temperature Coefficient Determination at Startup Af ter Refueling", reviewed July 30, 1979 had a calculational worksheet enclosed without the performing individual initialing or dating his work.
4.
SP-317, "RCS Water Inventory Balance", reviewed August 8, 1979 com-piled data and calculated identified and unidentified leakage rates without the compiler (s) initialing and dating the work.
This is a deficiency.
