Based on the resu1ts of an NRC inspection conducted on May 22 and 23,

1975, it appears that certain of your activities were not conducted in full conpliance with NRC regulations and conditions of your license Itens la and Ib are considered to be Infractions, as indicated below.

Power Operations Procedure QGP 3-1, Rev.1, sets the linits for the linear heat generation rate based on preconditioning a.

Contrazy to the above, the actual linear heat generatien rate on May 21 and 22 was approxinately 20 to 45 percent greater than the w aviam allowed.

10 CFR Part 50 Appendix B, Criterion V, " Ins.tructions, Procedures and Drawings", requires that activities affecting quality shall be prescribed by instructions, procedures and drawings and shall be accomplished according to these instructions, procedures and drawings. Section 13.3 of the FSAR. states that the Procedures Manual vill be prepared to cover all nornal and reasonably fore-sceable abnor=al operating conditions. Mninistrative procedure QAP 39-3-4 requires that orders for other than normal operations shall be written in the Daily Order Book to cover periods of time when the Operating Engineer is offsite.
Contrary to this require =ent, instructions given to the operating staff regarding other than normal operations (a change in rod with-draval sequence) on the second shife on May 21, 1975, were not for-nally documented in the Daily Order Book, and the Operating Engineer was offsite when such operations were perfor=ed.
Civil Penalty $2,000 This notice is sent to you pursuant to the provisions of Section 2.201 of the NRC's " Rules of Practica", Part 2. Title 10, Code of Federal Regulations. Section 2.201 requires you to submit to this office within twenty (20) days of your receipt of this notice a written state-cent or explanation in reply including (1) corrective actions stich have been taken by you and the results achieved; (2) corrective actions which vill be taken to avoid further items of nonco=pliance; and (3) the data.
