The exact cause of this occurrence has not been determined.

LS511 was verified to be l

[ g] ] operating properly. A similar eveut occurred on 11/25/78 and was reported in LER

[, p l78-083/03L-0. A test procedura has been developed and will be performed to simulate l

[ g l the conditions that resulted in both occurrences.

A supplemental report will be sub-l RT] l mitted to LER 78-088/03L-o upon completion of the investigation.
