SUPPLEMENTARY INFORMATION Report No.: 50-302/79-098/99T-0 Facility: Crystal River Unit #3 Report Date: 31 October 1979 Occurrence Date: 21 October 1979 Identification of Occurrence:

Triaxial time-history accelograph SI-2-MAT inoperable greater than thirty (30) days contrary to Technical Specifica-tion 3.3.3.3 and reportable under Section 6.9.2.b.

Conditions Prior to Occurrence:

Mode 1 power operation (100%).

Description of Occurrence:

At 1900, during routine power operation, it was discovered that the triaxial time-history accelograph SI-2-MAT, located on the containment ring girder was improperly mounted follow-ing a previous maintenance activity. Further investigation revealed the the accelograph had been remounted on a hold-down nut that resulted in the accelograph being elevated above its mounting base. Maintenance restored operability on 30 October 1979.

Designation of Apparent Cause:

The cause of this event is attributed to personnel error.

SI-2-MAT was improperly mounted following a previous mainten-ance activity.

Analysis of Occurrence:

No effect upon the plant or general public.

Corrective Action:

Technical maintenance personnel have been instructed of proper accelograph installation techniques.

Failure Data:

This is the first occurrence of this type reported.
