" performance of structures, systems, or components that require remedial action or ccrrective measures to prevent. operation in a manner less conservative than assumed in t!'e accident analysis in the sa fety analysis report..."

Descrintion of the Event:

During the seismic anchor inspection program being per-formed in response to IE Bulletin 79-02, a rigid hanger, 3-14-HB-S21, containing two shall type anchor bolts was found detached from the wall.

This hanger is associated with the "B" loop of the Unit 3 Core Spray piping.

The two anchor bolts were replaced with two wedge type anchor bolts.

The safety significance of this event is small because Unit 3 reactor is in the cold shutdown condition for refueling.

The system associated with this hanger, "B"

loop of the Unit 3 Core Spray piping, need not be operable under these conditions per Technical Specification 3.5.F.3.

Cause of Event

The most probable cause of failure foc these tuo anchor bolts was improper installation.

Corrective Action

The two failed shell type anchor bolts were replaced with two wedge type bolts. Corrective action has been completed.
