Observed inadequacy in implementation of administrative or pro-cedural controls.

Conditions Prior to Occurrence:

The reactor was in the "Run" mode at 119 0 'L't.

Description of Occurrence:

Technical Specification Table 3.2.F was not cet, in that the ref-erence chamber vent valve for PC-LT-12 (level transmitter) was lef t closed after the co=pletion of a special test on HPCI-LS-91B (level switch) upper shutof f valve.

This caused PC-LI-12 (level indica-tor) to give a high water level indication in the torus.

The erroneous indication was noted and corrected approximately 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> after the valve had been left closed.

Designation of Apparent Cause of Occurrence:

The technician failed to open the reference chamber vent valve for PC-LT-12 af ter verifying that HPCI-LS-91B upper shutoff valve was open and free of obstruction.

Analysis of Occurrence:

Level transmitter PC-LT-12 provides one of two narrow range torus water level indications.

Two wider range torus water level indi-cations are also provided. The redundant torus level indications were operable and the torus water level was at the proper level.

Reactor Technical Specifications permit one of the redundant level indicators to be out of service for 30 days.

This occurrence presented no adverse consequences from the standpoint of public health and safety.

Corrective Action

The reference chamber vent valve for PC-LT-12 was opened placing PC-LT-12 and PC-LI-12 back inta-service.

This occurrence has been discussed with the personnel involved and a copy of the nonconformance report has been routed to all Instru-cent Technicians.
