SUPPLEMENTARY INFORMATION REPORT NO.: 50-302/79-102/03L-0 FACILITY: Crystal River Unit #3 REPORT DATE: 15 November 1979 OCCURRENCE DATE: 23 October 1979 IDENTIFICATION OF OCCURRENCE:

"B" Core Flood Tank Sample Isolation Valve CFV-12 inoperable contrary to Technical Specification 3.6.3.1.b.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Power Operation (100%)

DESCRIPTION OF OCCURRENCE:

At 0600 following routine sampling of "B" Core Flood Tank, it was reported that "B" CFT Sample Isolation Valve CFV-12 would not close. CFT Sample Isolation Valve CFV-42 was closed and disabled to isolate the affected penetration immediately.

CFV-42 operability was restored when CFV-12 was closed. CFV-12 is being maintained in the closed position with the valve operating power supply de-energized and under administrative control until repairs can be completed.

DESIGNATION OF APPARENT CAUSE:

The cause of this event is unknown. Troubleshooting and subsequent repair of CFV-12 is being delayed due to the inability to perform required post-maintenance leak-rate testing. The leak-rate test equipment is presently under repair.

ANALYSIS OF OCCURRENCE:

No effect upon the plant or general public.

CORRECTIVE ACTION:

Further investigation into the cause of this event will commence upon completion of repairs to the leak-rate test equipment. Corrective action will be based upon the results of that investigation and submitted as a supplement to this report.

FAILURE DATA:

This is the second occurrence reported for CFV-12. Reference LER 77-109.
