i APPENDIX A NOTICE OF VIOLATION License No. DPR-57 Georgia Power Company NPF-5 E. I. Hatch, Unit I and Unit 2 Based on the NRC inspection March 13 - April 6, 1979, certain of your activi-ties were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized ss described in corre-spondence to you dated December 31, 1974.
A.
Technical Specification 3.7.C.1 requires that the integrity of the secondary containment shall be meintained.
10 CFR 50, Appendix B, Criterion XVI, implemented by Plant Hatch Quality Assurance Manual, Section 16, Paragraph 16.1, states in part that " Measures shall be established to assure that conditions adverse to quality... are promptly identified and corrected," and "in case of significant condi-tions adverse to quality, the me.asures shall assure that the cause of the condition is determined and corrective action taken to preclude repetition."
Contrary to the above, secondary containment integrity has been repeatedly compromised (17 times) since 1976 as reported in Licensee Event Report (LER) 50-321/79-15. The failure to correct this long tern problem con-stitutes ineffective corrective action.
This is an infraction and is applicable to Unit 1.
B.
Technical Specification Table 3.3.2-2, Item 4.c., requires that the High Pressure Coolant Injection (HPCI) turbine isolate at less than or equal to 10 psig erbaust diaphragm pressure. 10 CFR 50, Appendix B, Criterion XVI, implemented by Plant Hatch Quality Assurance Manual, Section 16, Paragraph 16.1, states in part that " Measures shall be established to assure that conditions adverse te quality... are promptly identified and corrected," and "in the case of significant conditions adverse to quality, the measures shall assure that the cause of the condition is determined and corrective action taken to preclude repetition."
Contrary to the above, when it was discovered in December, 1978, that the I was calibrated HPCI turbine exhaust diaphragm pressure instrument on Unit with a head correction of 13.8 psig on a dry line which should not have a head correction (LER 50-321/78-96), an immediate investigation was, not performed on Unit 2 for the same condition.
In March, 1979, approximately three months after the condition was noted on Unit 1, it was reported (LER 50-366/79-28) that the exact same condition was found to exist on Unit 2 (the 13.8 psig nonconservative error).
