APPEh3IX A NOTICE OF VIOLATION Duke Power Company License Nos. DPR-38 Oconee l and 3
& DPR-55 Baseo on the NRC ir.spection May 1-31, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by Technical Specification 3.9.8, sampling and activity analysis shall be performed prior to releasing liquid waste.
Contrary to the above, on May 24, 1979, 2335 gallons of radioactive liquid waste were released from the "A" laundry and hot shower tank without prior sampling and activity analysis.
This is an infraction and is applicable to Unit 1.
B.
As required by Technical Specification 6.4.1.b, procedures shall be followed during refueling operations.
Contrary to the above, on or before May 24, 1979, valve 3LP-59 was opened without an approved procedure or an out of normal authorization and failure to properly control this valve contt buted to a radioactive liquid spill.
This is an infraction and is applicable to Unit 3.
