As required by Technical Specification 6.4.1.b, procedures shall be followed during refueling operations.

Contrary to the above, on or before May 24, 1979, valve 3LP-59 was opened without an approved procedure or an out of n.,rmal authorization and f ailure te properly control this valve contributed te a radioactive liquid spill.

Response

This valve and similar valves on Units 1, 2 were closed or verified closed as required by the system procedure.

In order to prevent recurrence of this event, several actions were 1::'tiated.
