SKAGIT NUCLEAR PROJECT QUALITY ASSURANCE My name is E. Victor Padgett, Jr. My present position with Puget Power is Director Compensation & Benefits. I held the position of Manager Quality Assurance from Septem-ber 1973 until May 1979. A resume of my work experience is provided by Attachment 1.

My testimony is being offered in response to the Licensing Board's request posed during the April 24, 1979, prehearing conference in Seattle, Washington.

As Manager Quality Assurance, I reported to the Vice President, Engineering and Construction, and was respons-ible for the development, administration, and implementa-tion of the overall Quality Assurance Program for the Skagit Nuclear Project. This organizational structure provided me with the authority and organizational freedom to perform my duties in compliance with Criterion I of 10CFR50 Appendix B.

I had stop work authority to control conditions adverse to quality until effective corrective action was taken and verified. I also had the authority to bring quality prob-lems to the attention of the President and Chief Executive Officer of the Company for resolution if they could not be resolved at a lower level.

for the 5 1/2 years prior to my protection, the Ouality Assurance organization consisted of myself and a Quality Assurance Specialist. As Manager Quality Assurance, I was responsib'a for directing the development, administration, and impli .1tation of the overall Quality Assurance Program.

The Quality Assurance Specialist was responsible for provid-ing administrative assistance, preparing, and maintaining the Project Quality Assurance Manual and instructions, per-forming Quality Assurance indoctrination and training, and perforcing audits.

Puget's Quality Assurance Program was developed to meet the requirements of 10CFR50, Appendix B, and ANSI N45.2 as set forth in Chapter 17 of the PSAR for the Skagit Nuclear Project.

The Puget Power Quality Assurance Program consists of three tiers of documents. The first level document is the Quality Assurance Manual, which describes the requirements; designates the organizational responsibilities; sets forth the policy positions of Puget Power; and establishes the governing principles and framework for the total C ality Assurance Program. The second level of documents consists of organizational procedures that specify the method by which each organization implements the provisions of the Quality Assurance Manual. The third level consists of department directives and instructions.

The major efforts of the Quality Assurance organization when I was Manager involved design control; procurement document control; control of purchased material, equipment and services; and audits. Besides developing the Quality Assurance Program that governs Puget Power's own work, we were involved in setting up Bechtel's Quality Assurance Program for the Skagit Nuclear Project to assure that it was consistent with our commitments and approach.

We also evaluated Bechtel's implementation of their program, including assessment of Bechtel's system of source evaluation and selection. In addition, we participated in some quality assurance program audits of suppliers, as well as hold and witness point inspections at suppliers' facilities. Our audits also evaluated Bechtel and its sub-contractor's maintenance and storage facilities that are housing the equipment that has already been received.

A Nonconformance Report (NCR) and Corrective Action Request (CA' System was developed and implemented to assure that nonconforming materials are controlled to prevent their inadvertent use or installation and that prompt effective corrective action is taken when conditions occur that could adversely affect quality.

PROFESSIONAL QUALIFICATIONS My name is E. Victor Padgett, Jr. I have been employed by Puget Power since September 1973. I was Manager Quality Assurance until May 1, 1979, when I became Director Compen-sation and Benefits.

I have 23 years of experience in nuclear work, 16 of which have been in supesvisory or management positions involving process control or quality assurance. Twelve years of this experience was in the position of Manager Quality Assurance or Quality Control. I began as a process engineer working for General Electric at Hanford where I spent nine years in process control engineering, including responsibilities as a process engineering supervisor. In 1965, I left General Electric and joined United Nuclear Corporation, serving first at Hanford and later with the Naval Productc Division and also the Chemical Operations divisions. While working for United Nuclear, I served as Manager Quality Control for two different divisions, and also as Manager Operations.

I have a B.S. degree in Chemical Engineering from Oklahoma State University. I have also attended the General Electric School of Nuclear Engineering and the University of Connec-ticut Quality Control Management Institute. I am a former member and Chairman of the Richland Section of the American Society for Quality Control.
