SUPPLEMENTARY INFORMATION Report No.: 50-302/79-069/03L-0 Facility: Crystal River Unit #3 Report Date: 16 August 1979 Cccurrence Date: 24 July 1979 Identification of Occurrence:

"B" Emergency Diesel Generator inoperable contrary to Technical Specification 3.8.1.1.

Conditions Prior to Occurrence:

Mode 4 hot shutdown.

Description of Occurrence:

At 0700 during the performance of SP-354, it was reported that "B" Emergency Diesel Generator had excessive exhaust manifold temperatures and fire exiting the exhaust muffler. The Diesel Generator was rendered inoperable. Trouble-shooting and testing revealed the following:

1) Engina operation appeared " rich"
2) Air leak noted on turbocharger discharge
3) Exhaust manifold leakage
4) Higher than expected cylinder and exhaust manifold temperatures.

Maintenance actions were initiated and "B" Emergency Diesel was returned t- service at 2330, 25 July 1979 following satisfactory completion of SP-354.

Designation of Ap,arent Cause:

The cause of this event, is attributed to fuel accumulation in the exhaust manifold resulting from a " rich" fuel mixture caused by an air leak in the turbocharger discharge.

Analysis of Occurrence:

No effect upon the plant or general public as the plant remained in a safe shutdown condition.

Corrective Action:

Repairs were initiated to prevent leakage on the exhaust manifold and turbocharger discharge. As a result of these repairs, diesel exhaust temperatures and engine cylinder temperatures were corrected.

Further testing displayed satisfactory operation of the diesel and the diesel generator was returned to service.

Subsequent preventive maintenance and inspection activities will be performed as soon as possible upon receipt of repair parts.

Failure Data:

This is the first occurrence of this type reported.
