1. DESCRIPTION OF EVENT:

Operations was unable to establish a satisfactory control room pressure differential while performing MOP. 21. 3, control room and relay room operational pressure test. Investigation revealed that damper MOV-VS-104B was not fully closed. This is contrary .to Technical Specification 3.19A and is reportable per Technical Specification 6.6.2.B.(2). The MOP was deviated to use this damper and the associated fan, 2-VS-F-41, to verify compliance with Technical Specification 3.19.B~

2. PROBABLE CONSEQUENCES AND STATUS OF REDUNDANT EQUIPMENT:

As shown by the testing of the emergency ventilation system, the air bottles would not have been able to maintain the required pressure differential.

However, the control room and relay room pressure was maintained by the emergency ventilation system (T.S.3.~9.B). The health and safety of the public were not affected.

3, CAUSE:

The clamp holding the damper's operating arm had slipped.

4. IMMEDIATE CORRECTIVE ACTION:

Verified that the control room emergency ventilation system was functional, as required by Technical Specification 3.19.B.

5. SUBSEQUENT CORRECTIVE ACTION:

Adjusted MOV-VS-104B to insure full closure, and subsequently performed a satisfactory Control Room Pressure Test.

6. ACTION TAKEN TO PREVENT RECCURANCE:

None required.

None.
