DESCRIPTION OF EVENT

e Im.properly Installed During the performance of 1-PT-39B-1.~ Inaccessible* Snubbers Visual Inspection, snubbers l-Rc-nss..:116 and l-RC-HSs*...,117 were discovered in-stalled improperly, i.e. 90° out of 'proper position.

The snubbers were declared inoperable *. This event is reportable.in accordance with Tech.

Spec. 6.6.2.b.2.

2)

PROBABLE CONSEQUENCES The snubbers were installed with the reservoir below the valve block.

This could have caused air

  • introduction into the. valve *block or cylinder thus preventing the snubbers from performing their intended function~

However, since no seismic disturbances were encountered while the Unit was at power, the health and safety of the public were not affected.

3)

CAUSE OF EVENT

The snubbers were installed improperly due to a.deficiency o*f tbe design change utilized to correct pipe support deficiencies discovered under IE Bulletin 79-14.

4)

IMMEDIATE CORRECTIVE ACTIONS

The snubbers were declared inoperable.

5)

SUBSEQUENCE CORRECTIVE ACTIONS The snubber end mounting bracket* wi:11 be.removed,

  • rotated 90° and reinstalled.

The snubber will be purged :and reinstalled in the proper orientation_ prior *to Unit Startup.

6)

ACTION TAKEN TO PREVENT RECURRENCE Design personnel have been apprised of the importance of proper snubber orientation.

7)

GENER.IC IMPLICATIONS The Vi~ual Inspection performed at Unit shutdown covers all snubber installations.

Since no other snubbers were discovered installed improperly this event possesses no generic implications.
