Minimum Wall Thickness Variance in Reducers to MOV-1842

  • 1.

Decription of Event:

With the unit at cold-* shutdown, a. possible minimum wall thickness violation was discovered iri the reducers mating with MOV-1842.

,Infor-mation provided by the Architect-Engineer determined that the minimum wall thickness had been violated.

This event is reportable in accordance with Technical Specification 6.6.2.a.2.

2.

Probable Consequences/Status of Redundant Systems:

It was determined that the wall thickness of the 4" side of the reducers was 0.031" less than the required minimum thickness.

Since no problems were experienced with these reducers, the health and safety of the public were not affected.

3.

Cause

It appears that the reducers (as manufactured) would_ riot mate properly to MOV-1842 and during construction the reducers were machined to insure a proper fit with the valve.

In the course of this modification to the reducers, the minimum acceptable wall thickness was violated.

4.

Immediate Corrective Action

During the repair work on the MOV, the reducers were replaced and verified to meet the minimum wall thickness criteria.

5.

Scheduled Corrective Action:

The reducer replacement was made prior to the unit startup and no further action is deemed necessary.

6.

Action Taken To Prevent Recurrence:

Since the reducers for MOV-1842 were replaced with acceptable minimum wall thickness reducers, no further action will be taken to prevent recurrence.

7.

Generic Implications:

This event was an isolated occurrence with no generic -implications.
