Low Safety Injection Accumulator Level

Description of Event

With the unit at rated power, the unit operator noted a need to fill "B" and "C" safety injection accumulators.

• The hydro test pump was started and the*

accumulators were valved in to the fill header. *The operator, noting that the accumulator levels were not increasing, ordered a check of the hydro test pump.

The hydro test pump was reported to be lifting its rel~ef valve and ~he operator stopped the pump and closed the fill valves to "B" and "C" accumulators. It was then noted that "C" accumulator had transferred some of its water.to "B" due to differential pressure and had dropped 1% below the limits specified on Technical.*

Specification 3.3.A.2. This is reportable in accordance with Technical.

Specification 6.6.2.b.(2).

Probable Consequences/Status of Redundant Systems:

In the course of the fill operation, ~the water transfer :occurred, to another accumulator.

No water was lostfrom.~he accumulator system.

Technical Specification 3.3.B.l permits one accumulator to be out of service no more than four hours for corrective action.

The health and safety of the general public were not affected.

Cause of Event

The event occurred because of lifting of the relief of the hydro test pUI!lp whicq prevented the hydro test pump from building up discharge.pressure to allow filling.

The relief valve was removed and replaced&

The.hydro test pump and relief valve were vented and checked for set point.

Subsequent test of the relief valve and pump was satisfactory.

The "C" accumulator was filled to the proper level.

The accumulator was declared back in service 55 minutes after the level was ~oted low.

Subsequent Corrective Action:

None required.
