COMBINED INSPECTION REPORT NOS. 50-272/95-12 AND 50-311/95-12 AND
Dear Mr. Eliason:
This letter refers to the announced safety inspection conducted by
Mr. L. Scholl of this office from May 15 to June 8, 1995, at Salem Nuclear
Generating Station, Units 1 and 2.
The inspector discussed our findings with
members of your staff at the end of the inspection. Areas examined during
this inspection are described in the enclosed inspection report.
The inspection was directed toward activities associated with the operation
and maintenance of electrical circuit breakers and consisted of examination of
procedures and representative records, interviews with personnel, and
observations by the inspector.
Circuit breakers are important components in many safety systems and the
reliable operation of these components is essential to ensure the successful
accomplishment of syslem safety functions.
Deficiencies in the design,
manufacture, or maintenance of the breakers have the potential for introducing
a common mode failure that could result in the failure of multiple trains of
safety systems and result in the loss of a safety function.
Based on the results of this inspection, the NRC has determined that a
violation of NRC requirements occurred.
The violation is cited in the
enclosed Notice of Violation (Notice), and the circumstances surrounding it
are described in detail in the subject inspection report. The violation is of
concern because a significant number of circuit breaker failures have occurred
due to the same apparent root cause of hardened and/or dirty grease in the
operating mechanism.
In each case the corrective actions were narrowly
focused on the repair of the individual failure and the generic implications
and potential safety significance of a condition that could result in the
common mode failure of several components was not promptly addressed.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence.
Your response may reference or
include previous docketed correspondence, if the correspondence adequately

Notice, including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of
this letter, its enclosures, and your response will be placed in the NRC
Public Document Room (PDR).
To the extent possible, your response should not
include any personal privacy, proprietary, or safeguards information so that
it can be placed in the PDR without redaction.
The responses directed by this letter and the enclosed.Notice are not subject
to the clearance procedures of the Office of Management and Budget as required
by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511. ,
In addition to the circuit breaker inspection, the inspectors reviewed active
operability determinations for Salem Units 1 and 2.
The inspectors found the
determinations to be of a variable quality and some of the evaluations had
weak documented technical bases.
In several cases, actions necessary to
monitor degraded conditions to ensure continued operability were not clearly
specified.
